Please complete all the sections on the form. The registration section allows you to complete the details from the registration form and request accommodation (if required). Indicate details of how you intend making payment before entering the secure site for on-line payment.

Click SUBMIT, and the form will be transmitted to the Conference Secretariat. You will receive an on screen confirmation of your submitted registration details and an email confirmation once we receive the registration.

Please note your registration cannot be confirmed in writing until payment is received at the Conference Secretariat. To qualify for any earlybird rate, payment must be received at the Secretariat before the date shown or the full amount will be charged.

Payments may be made by cheque or credit card. Cheques (or money orders) may be forwarded by mail - please include a copy of your registration details.

Payment by credit card is available on the registration form or on-line.

Alternatively, you may fax a copy of the registration form with details of your credit card.
